Syphilis Testing Explained

Syphilis is sometimes thought of as a disease of the past, but cases in the UK have been rising steadily in recent years. The reassuring news is that it remains straightforward to detect with a blood test and is easily cured with antibiotics when caught. Testing is a routine, confidential part of looking after your sexual health, and understanding how it works takes the worry out of it.

How syphilis progresses

Syphilis is a bacterial infection that develops in stages if untreated:

• Primary stage — a usually painless sore at the site of infection, which can easily be missed.
• Secondary stage — a rash, often on the palms and soles, along with flu-like symptoms.
• Latent stage — a symptom-free period that can last for years.
• Late stage — rare today, but potentially serious if the infection is left untreated for a very long time.

Because the early signs can be subtle or absent, many people are unaware they have it — which is exactly why a blood test is so useful.

How the test works

Syphilis is detected with a simple antibody blood test, which looks for the immune response to the bacteria. It is quick and reliable, and forms part of most complete STI screens. Antibodies can take up to around twelve weeks to develop after exposure, so timing your test appropriately gives the most dependable result.

Why testing matters

Early diagnosis makes treatment simple — usually a course of antibiotics that clears the infection completely. It also prevents onward transmission and avoids the rare but serious complications of very late, untreated infection. For pregnant women, testing is especially important, as syphilis can affect the baby if not treated.

What a positive result means

A positive antibody result means you have been exposed to syphilis at some point and need assessment to determine whether treatment is required — sometimes it reflects a past, already-treated infection. When to get tested

Consider testing with a new partner, after unprotected sex, as part of an annual sexual health check, or if a partner has been diagnosed.
